Examples of the Difference We Make

Monument Impact received a $500,000 grant to support HEAL Zone activities in the Monument community, such as improving snack and beverage options at local youth baseball games; a walking audit that led to sidewalk and signal changes; implementing Zumba® and cooking classes; and, improving nutrition and physical activity options a local childcare settings.

 

In east Contra Costa County, Contra Costa Health Services received $50,000 to support Healthy Families America, which provides home visitation for pregnant and post-partum women, with a focus on African American

Improve Access to Care

Increase access to health care and coverage for low-income and uninsured people – assuring adequate availability in Spanish.

Obesity icon

Increase Healthy Eating and Active Living

Reduce consumption of sugar-sweetened beverages, improve access to fresh fruits and vegetables, and promote physical activity in schools and community based settings. Educate and inspire children, teens, and adults to make healthier choices.

Asthma icon

Increase Asthma Prevention and Management

Help people get screened and referred to health services, reduce health emergencies resulting from asthma, and increase awareness about the impact of pollution while advancing management of asthma in community based settings.

Access to Care icon

Increase Access to Peri-Natal Care

Help low-income women get access to and information on pre-conception, pre-natal, and post-partum services and self-care.
